Introduction :
Colorectal cancer is the 3rd most common cancer worldwide. From Hong Kong Cancer Registry in 2019, colorectal cancer case is the 2nd commonest cancer. Colostomies/Stomas are part of the therapeutic treatment of colorectal cancer. It causes problems for both physical and psychosocial functioning and affect quality of life by altering the patient’s self-image, defecation pattern and lifestyles Patient having a stoma with negatively impact on self-efficacy and the health-related quality of life in colorectal cancer survivors. The development of psycho-education program is significant to focus and improve on the stoma care self-efficacy and quality of life of colorectal cancer patients with stoma formation after surgery and fill the clinical gap.
Objectives :
The aim of this study to examine the effectiveness of psycho-education program for colorectal cancer patient with stoma formation after surgery. The objectives are to investigate the stoma care self-efficacy and health-related quality of care for colorectal cancer patient with stoma formation after surgery in Hong Kong.
Methodology :
This study is two-armed, single blinded randomized controlled trial. All eligible participants are enrolled and randomized into experimental or control group through permuted block randomization with a block size of 4 and 6. The inclusion criteria for this study are patients with stoma formation after colorectal cancer surgery; aged ≥18 years old; able to communicate in Cantonese and read Chinese; available to be contacted by telephone. The exclusion criteria are patients who are receiving treatment for mental disorders; diagnosis of cognitive impairment e.g., dementia; refused to participate in this study. For the intervention group, the psycho-education program will deliver by researcher with qualification of Enterostomal Therapist; the setting is in the Department of Surgery of PMH. The primary outcomes would be evaluated by monitoring changes in the stoma care self-efficacy of participants; using the Stoma Self-efficacy Scale (SSES) and secondary outcomes on health-related quality of life by using the Chinese (HK) version of the Short-Form Health Survey 36 (SF-36).
Result & Outcome :
The intervention group had significantly higher SSEC scores on aspects of stoma care, psycho-social, and lifestyles. Both the intervention and control groups had increasing SSES scores over time, and the improvement was significant. The intervention group also had significantly higher health-related quality of life when compared with the control group. The finding showed that the development of psycho-education program is significant to improve on the stoma care self-efficacy and quality of life of colorectal cancer patients with stoma formation after surgery.
