Smart use of QR code to enhance visual acuity service in Kwai Chung district General Out-Patient Clinics (GOPCs) of Kowloon West Cluster (KWC)

Introduction :
Smart hospital and smart clinic is a trend in recent years. Technology can improve the clinic workflow, enhance the efficiency of work, improve the communication between multidiscipline.

Every client will perform visual acuity (VA) test on booking date when there is a referral of optometrist service. Previously, patients’ VA results were written on the optometrist appointment slip. With the implementation of HA GO, patient started not to bring along them appointment slips upon registration. Baseline VA test result could not be retrieved. On the other hand, patient need to perform another VA test on appointment date which duplicated the workload.

A QR code was created in this pilot program and implemented in 3 Kwai Chung district GOPCs ( Ha Kwai Chung GOPC, South Kwai Chung GOPC and North Kwai Chung GOPC) starting from April 2023. Nurses and patient care assistants enter the VA result in DMCS template in CMS by scanning a QR code. Once the patient attends the optometrist service appointment, VA result can be retrieved from CMS instead of hard copies.
Objectives :
- standardize the format of nursing template through accessing a QR code
- enhance the efficiency and compliance of entering VA result to prevent data loss
Methodology :
Generate the QR codes to enhance the compliance of entering VA result. Outcome measures were determined by data collection of optometrist service attendance and VA data in CMS during the period of 4/2023-3/2024.
Result & Outcome :
The VA service was improved by:
- Saving time to type VA result format, the format is simply generated by scanning the QR code.
- Multi-discipline can obtain VA result in DMCS so as to facilitate their management
- Reducing the chance of data loss and duplicated workload on performing VA assessment again.

There were total 3993 attendances in the pilot period. At the initial stage in 4/2023, there were 10.6% data loss. The condition improved after 1 month implementation of the program, data loss reduced to 5.6% in 5/2023, 1.9% in 6/2023, and 0% in 3/2024.
