Introduction :
With a notable number of Pressure Injuries (PI) Hospital Acquired (HA) Rate in United Christian Hospital, the measures of PI prevention should be addressed. To tackle this challenge, early detection and timely intervention are fundamental pillars of proactive to prevent PI. By identifying potential PI in early stage, nurses can implement timely intervention and improve the treatment outcomes.
Objectives :
 Early Detection - To enhance the knowledge of PI and related primary care for Patient Care Assistants (PCAs) as to foster the awareness, knowledge application, and reporting practices of PI.  Timely Intervention - To promptly intervene and manage the potential PI cases by nurses. - Using suitable new technological pressure relief products to facilitate the simple and effective care for PI prevention.
Methodology :
 Early Detection - Two identical 20-minute PowerPoint education sessions for PCAs were conducted in March 2024. The content mainly focused on basic PI knowledge and related primary patient cares intervention. - To enhance the communication within healthcare team by setting up a “Pressure Injury Reporting Record Broad”  Timely Intervention - Daily PI nursing round and case discussion among Duty IC and case nurses. - Applying preventive measures for potential PI / Stage 1 PI cases. - Introducing new pressure relieving inflatable mattress & heel free heel-protectors.  Evaluation 1. Pre- and Post-Education Knowledge Assessment A single-group, pre- and post-test (8 MCQs with passing score 4/8) was conducted to evaluate the effectiveness of the staff education. 2. Reporting Awareness A one-month time comparison of the number of Stage 1 PI case reporting before (February,2024) and after (March,2024) the education session. 3. Effectiveness of Preventive PI Management Reviewing the trend of Nursing Quality Index (NQI) of PI quarterly in 2024.
Result & Outcome :
All ward PCAs (n=12) attended the education sessions. The pre-test result showed that 66% of failed (Mean Score 0.38), while 100% passed the post-test (Mean Score 0.89), demonstrating significant knowledge improvement. There was no Stage 1 PI case reported by PCAs in Feb 2024. After the education session (March 2024), 4 Stage 1 PI cases were reported by PCAs and confirmed by nurses; then recruiting into Daily PI nursing round for further observation and management(i.e., Applied new pressure relieving inflatable mattress).
The NQI of PI showed a decreasing trend in figures (2Q2024 1.45 > 3Q2024 0.82 > 4Q2024 0.67). Moreover, it was lower than the hospital benchmark. The results revealed that this Preventive PI Management had a significant and sustainable effect.
