Introduction :
• Hematology patients are immune-compromised after intensive chemotherapy as high fall risk patient group
• Complications of novel agent and treatment strategy may cause unforeseen complication including anemia, fatigue and muscle loss which are potential risk factors of fall.
• Fall prevention education is an essential part of routine for new admission patient that could identify patient’s self-perception of his /her own potential fall risk and empower patients to reduce risk of falling through education.
Objectives :
• To enhance the accuracy for Morse Fall Scale (MFS).
• To help nursing staff to identify those patients are overestimates or forgets their limitations therefore nurses are able to provide corresponding preventive measures and educations.
• To save nursing care time (15 mins) on repeating fall prevention information including ward orientation, risk factors of fall during hospitalization etc.
Methodology :
• Produce an education video on Fall Prevention for haematology patients
• Let patients see the educational video upon admission
• Help patients identify their own fall risks (common in post chemotherapy patients) and provide corresponding education.
Result & Outcome :
Through the education video, patient can learn more about their fall risks and the measures to prevent falls. Nurses can tailor education and preventive measures based on each patient’s specific weakness or deficits.
Conclusion:
1. Patient awareness and knowledge about fall risks could be enhanced, potential reducing the fall incidence rate. The fall rate decreased by 14% from 2022 to 2024.
2. Nurses could save approximately 15 mins per admission case by reducing the time need to introduce fall prevention measures. With an average of three admission case per day, this could save 4.5 working hours per week.
