Integrating Tai Chi and Hydrotherapy: A Novel Approach for Chronic Respiratory Disease Rehabilitation

Introduction :
Ai Chi is a novel hydrotherapy approach that integrates Tai Chi principles with hydrotherapy techniques to enhance the well-being of patients with chronic respiratory diseases. Substantial evidence demonstrates that hydrotherapy significantly improves dyspnea, exercise capacity, lung function, respiratory muscle strength, and quality of life. Ai Chi leverages the unique properties of water alongside with Tai Chi characteristics of mindfulness, breathing techniques and circular body movement to improve both physical and mental health may further enhance the rehabilitation outcomes.
Objectives :
To formulate and implement a comprehensive Ai Chi program for patients with chronic respiratory diseases
Methodology :
Preparation of Ai Chi program since April 2024, it included program planning, resource preparation, risk management, and patient recruitment. The program comprised of 19 movement styles that emphasized breathing, coordination, core stability, and balance, with sessions conducted bi-weekly over an eight-week period. Pre-&post-program evaluation included functional capacity, respiratory muscle strength, ventilatory efficacy and quality of life. To facilitate understanding and participation, videos and exercise pamphlets demonstrating underwater Ai Chi movements were provided to participants prior to the first session. Risk management strategies ensured proper ventilation, temperature control, and the availability of oxygen concentrators with nasal cannulas/masks for oxygen-dependent patients. Additionally, a post-program questionnaire will be administered to assess patient satisfaction and engagement.
Result & Outcome :
The first session of Ai Chi program commenced on July 31, 2024, with nine patients enrolled and recruitment was ongoing. Preliminary feedback indicated high satisfaction with the logistics of the program and the arrangement of the resources especially the video on underwater Ai Chi movements. Participants appreciated the physiotherapists' professional assessments and clear explanations of the program structure, risks, and benefits. Adequate temperature control, ventilation, and equipment readiness contributed to a safe and comfortable environment. Besides, music and floats were incorporated to enhance the enjoyable atmosphere during exercise. Although two participants experienced dyspnea necessitating frequent breaks during initial sessions, they exhibited gradual improvement with continued training. The initial phase of the Ai Chi program successfully engaging the participants in underwater exercises with positive feedback. The participants can practice the exercises at their convenience at public swimming pool, fostering regular routines and better self-management, which reinforces its value as an alternative after the program. Quantitative assessments are forthcoming to further evaluate the participants' improvement.
