Introduction :
Team communication is important in provision of safe and effective nursing care. However, survey was conducted to those new nurses in UCH around the end of their first year nursing career in 2023. Communication was the top self-perceived weakness throughout their nursing experience. Communication is a complex and dynamic skill. It is difficult for new nurses to differentiate between effective and ineffective team communication.
Objectives :
Facilitate new nurses to: - Develop effective team communication skill to ensure patient safety - Enhance clinical competence on team communication in their first year experience
Methodology :
Simulation approach was adopted in different phases of preceptorship program to achieve the stated objectives, which was facilitated by trained simulation instructors in Clinical Teaching Team of UCH Phase 1: Simulation-based Clinical Handover & Nursing Documentation Workshop (0 month) This workshop was blended in the Orientation Program to guide new nurses in adopting the iSBAR protocol during clinical handover and build up self-awareness of good handover practice. Scenarios were designed with background of different specialties to facilitate participants to incorporate the iSBAR protocol in real situation. Phase 2: Simulation-based Team Dynamic Communication Workshop (3rd month) Being a case nurse required communication among multi-disciplinary team in complex clinical situation. Crew Resource Management (CRM) Classroom Training was arranged for developing communication skills of new nurses. Afterward, a half-day Simulation-based Team Dynamic Communication Workshop was designed to facilitate the application of CRM concepts through different simulation scenario. Phase 3: Simulation-based Advanced Clinical Skill Workshop (9th month) Apart from advanced clinical knowledge and skill, exercising effective teamwork and communication skill were also one the major objectives in this workshop. This promotes reflection from junior nurses in handling critical situation in team approach.
Result & Outcome :
Evaluation was conducted with satisfaction score as below. 1. Simulation-based Clinical Handover and Nursing Documentation Workshop: 5.56 out of 6 2. Simulation-based Team Dynamic Communication Workshop: 5.32 out of 6 3. Simulation-based Advanced Clinical Skill Workshop: 5.21 out of 6. An Adapted Clinical Competency Questionnaire (CCQ) was conducted to measure clinical competence of new nurses after each workshop. There was an increasing trend in mean score on the questions related to team communication 'Communicating verbally with precise and appropriate terminology in a timely manner with healthcare professionals' - Mean score increased from 3.16 (0th month) to 4.08 (9th month). 'Performing shift report (handover)' - Mean score increased from 2.47(0th month) to 4(9th month). Remark: CCQ mean score higher means higher level of clinical competence.
