Introduction :
Since the introduction of smart hospital projects in the Hospital Authority, taking clinical photos serve as supplementary information to track patients skin conditions besides the existing documentation charts. Good quality clinical photos with valid patient identities is mandatory in accurate data collections for patient management. This is also a big move in enhancing nursing initiative in smart hospital projects in Hospital Authority
Objectives :
The goal of the project is to perform effective clinical photo taking process with correct patient identities and good quality clinical photo taken. The project will focus on promoting users who perform clinical photo taking via two key steps in the photo taking process to justify patient identity and ensure photo qualities.
Methodology :
The introduction of two key steps in verification of clinical photo taking help colleagues to uphold the good practice in proper documentation. The first key step is to assess the Clinical Camera App (CCA) by scanning the 2D QR code wristband of the patient for patient verification. The second key step is to include patient’s name label (Label with name and Hospital number (HN) only) and a standardized ruler (e.g. a large size Tegaderm with ruler) into the clinical photo. Users should review the quality (correct focus and brightness) of the photos taken, verify patient identifiers in the photos with the system, mark sensitive photos if needed before uploading to the CMS. The two key steps verification process improves patient safety and increases the data accuracy.
Result & Outcome :
All the uploaded clinical photos can be reviewed in CMS with reference to the patient’s identifiers in the photo (i.e. the name label) and the scaling of the skin conditions can be clearly shown (I.e. the ruler). Different parties can review, document and trace the skin condition progress throughout the patient album in CMS. Clinical photos plays a major role for referencing the skin changes during the patient management process. The project can enhance patient’s safety and improve the effectiveness in clinical management journey.
