Introduction :
Incident of eye drop instillation (wrong eye) occasionally happened due to multiple factors such as environment distraction and too hurry in particular for those new nurses. O&VS Department plans to design/ use of a "simple' & "effective" Visual Management Tool (eye catching label) to enhance medication safety in particular for those procedures suites: Eye Day Ward/ OT / Laser Room/ Procedure Room etc. With the new tool, the "thinking" process can be leaned. Staff directly visualize the laterality of eye and instill eye drops to prescribed eye directly. Incidents of eye drop related to laterality hopefully can be minimized.
Objectives :
1. To design a visual management tool to enhance medication safety in Eye Day Ward & Eye Centre
2. To minimize incident of wrong eye drop instillation
3. To reduce "thinking" process and instill eye drops to prescribed eye directly
Methodology :
O&VS Department to design the eye catching label include: design the symbol to draw attention; select different colors to indicate left and reight eye to create a strong impact. Then paste the label onto the wall and patient chair in Eye Day Ward/ Eye Centre to enhance nurses' awareness during eye drop instillation. All nurses should visualize the laterality of the eye and instill eye drops to prescribed eye directly.
Result & Outcome :
After pasting the eye catching label onto the wall and patient chair in Eye Day Ward/ Eye Centre in 4Q 2024, that no more incidents occurred. Nurses were satisfied with this visual management tool
