Introduction :
In 2023, there were 14 fall incidents in PMH O&T department. However in 1Q 2024, there were 7 fall incidents happened, 5 cases occurred at night time. One case was complicated with head injury and twitching, GCS dropped from 15/15 to 12/15. According to the increasing trend of fall incident, CQI program on prevention of fall was carried out.
Objectives :
1) To raise patient and relative awareness on fall prevention strategies 2) reduce fall incident rate
Methodology :
(1) Fall prevention educational leaflet was designed to provide to patients and relatives on admission in order to raise their awareness. (2) Prepare a video in Robot for fall prevention education during visiting hours. (3) Educate all staff (nurses and PCAs) to turn on bed alarm for new admission cases until further assessment done, bed alarm was allowed to discontinued for those assessed as low fall risk.(4) Compliance check on bed alarm utilization
Result & Outcome :
Upon the initiation of the project, all ward staff, including nurses and PCAs, received training on activating bed alarms for newly admitted and transfer-in patients. Nurses conducted fall assessments to determine the necessity for continuous use of bed alarms. Compliance checks on the application of bed alarms revealed that they were appropriately utilized for patients identified as high fall risk. The compliance rate for bed alarm utilization was deemed satisfactory, and ward staff demonstrated heightened awareness regarding the application of bed alarms for high-risk patients. Additionally, educational leaflets were provided to patients and their relatives upon admission, and the Robot was used to broadcast fall prevention videos during the visiting hours to enhance awareness of fall prevention strategies to patients and relatives. As a result of these initiatives, the fall incident rate was recorded at 3 cases during 2Q & 3Q of 2024.
