Reinforcement on self-care for patients and carers in managing hypertension through Smart App in primary health care setting

Introduction :
Hypertension is prevalent in HK. Structured self blood pressure measurement (SBPM) education can enhance patient’s self-management, drug compliance and improving overall blood pressure control. Keeping track of blood pressure trend can be effective in achieving blood pressure control. Since 4/2024, Hospital Authority (HA) enhance Artificial Intelligence (AI) technology in HAGO “MyHealth” which is simple and convenient, focusing on better organizing health data and monitoring the BP trend as well as strengthening patient’s self-health management.
Objectives :
1. To enhance patient’s competence in performing self-blood pressure measurement and strengthening coping skills in BP management 2. To encourage patient to use HAGO “My Health” for recording and tracking their home BP data 3. To enhance staff’s competence in educating and instructing patient to use HAGO “MyHealth”
Methodology :
A program is implemented in Nov 2024 at Family Medicine and General Outpatient Department (FM & GOPD) in Our Lady of Maryknoll Hospital (OLMH). Total of 5 identical training workshops of HAGO “MyHealth” App were arranged for nursing staffs, patient care assistants and clerical staffs. Pamphlet on technique of taking blood pressure and step of recording and tracking of home BP reading at HAGO “MyHealth” was designed and provided to patient. The educational video on instruction of self blood pressure measurement and application on HAGO “MyHealth” in the form of QR Code were displayed on pamphlet. Patient with hypertension bringing with home BP device for validation were recruited upon follow up in OLMH FM& GOPD. The recruited patient should have smartphone and be able to handle simple smartphone applications by themselves or by their caregivers. Upon consent to participate in the program, patient received a series intervention as below : - Patient were asked to perform home BP measurement as they usually practice at home . The skills and competence of self BP monitoring were assessed by nurse with a standardized assessment form which include verbal and practical procedures on self BP measurement. Then nurse provided individual education and instruction on self BP measurement skills and knowledge; with post education competence were assessed. - Patient were invited to download HA GO App and nurse or clerical staff educated and demonstrated to use HAGO “My Health” for recording and tracking their BP reading - To assess the adoption and patients’ satisfaction of using HAGO “My Health” by questionnaires
Result & Outcome :
- Total 61 staff including 15 nursing staffs, 29 Patient care assistants and 17 clerical staffs joined the workshop. 91.8% of the staff strongly agreed or agreed that the workshop can enhance their ability to guide patients in using the HAGO “My Health”. -Total 59 patients and 1 caregiver completed the self BP measurement competency assessment. For pre- education assessment, found only 36.7% patient could position the cuff correctly and only 16.7% knew the desired blood pressure control is <130/80mmHg. After receiving education and instruction from nurses, the performance of self BP measurement was dynamically improved with 98.3% could position the cuff properly and 100% answered the desired blood pressure control correctly. - From the questionnaire result, 56.7% of the patient had prior downloaded HAGO. It was surprisingly found that nobody using HAGO “My Health” to record their home BP readings among them. 94% of them expressed that they didn’t know HAGO having blood pressure recording function. - For those not yet download HAGO, only 53.8% of the patients agreed to download HAGO instantly. - After promoting and educating of HAGO “My Health” App, 100 % of patient expressed willingness to use HAGO “My Health” for recording and tracking their BP reading.
