Introduction :
Introduction: For patients admitted to Wong Chuk Hang Hospital for infirmary care, they require complex long-term care, and they often stay until the end of their lives. This is a big challenge for their significant others to go through the process of the last journey with the patients. A Life Story Book (LSB) is a personalized collection of photographs and other mementos which facilitates an open dialogue and discussion on Advance Care Planning (ACP) according to patient’s experience and preferences.
Objectives :
(1)to facilitate an open dialogue and discussion on ACP according to patient’s experience and preferences. (2) to provide a channel for nursing staff to recognize patient’s stories and establish patient centered care.
Methodology :
A program entitled "Application of the Life Story Book in Advance Care Planning" was conducted in Wong Chuk Hang Hospital from July to December 2024.Participants from the LSB program (1) Infirmary patients with active support from carers, (2) Clients from Dementia Day Care Centre, and (3) Nurses and care-related supporting staff were recruited in the evaluation phase. Outcome measures were (1) Number of participants with consent for ACP and (2) Satisfaction survey with using 5-point Likert Scale.
Result & Outcome :
11 patients and 46 health care workers returned their questionnaire. 6 out of 11 patients (55%) signed ACP. Satisfaction survey result (1) Expressed positive sentiments and satisfaction with LSB, (μ= 4.45, range = 3-5) (2) The LSB was perceived as beneficial for reviewing patient’s uniqueness and preferences (μ=4.50, range = 2-5) (3) The LSB showed effectiveness in enhancing communication, facilitating relationship-building, and understanding in ACP and patient’s preferences. (μ=4.34, range = 3-5) Conclusions: The process of preparing the LSB provides an outlet for expressing emotions that are often implicit or avoided in traditional Chinese culture, such as the initiation of ACP and discussion on after-death arrangement. LSB can enhance patient centered care and communication.
