Introduction :
Intra-hospital transportation is a potentially hazardous procedure in Intensive Care Unit(ICU). A transportation audit was conducted from November-2022 to-March-2023 in ICU, which revealed 18% adverse events during intra-hospital transportation, due to low compliance with transportation checklist, inadequate situational awareness and ineffective communication. Adverse events during transport may cause significant consequences for the patients. Therefore, a simulation training was developed to promote transportation safety of critically-ill patients.
Objectives :
- Increase compliance with the transportation-checklists.
- Improve situational-awareness and emergency-response during transportation.
- Enhance teamwork and communication.
Methodology :
The 3-phase training program was implemented from 1stMarch to 15thMay, 2024, followed by a transportation audit for review on 4th September 2024.
Phase 1: Simulation
A 30-minute simulation was conducted and evaluated with a checklist and a staff evaluation form to gather feedback and assess the effectiveness of simulation.
Phase 2: Post-simulation sharing
Post-simulation sharing sessions were provided to nursing staff, highlighting the strengths, weaknesses, recommendations and learning points.
Phase 3: Onsite audits
The onsite audits were targeted at Category 1 and Category 2 unstable intrahospital transportation. Transportation checklists were collected for result analysis.
Result & Outcome :
1.The 3-phase training program:
Nurses, doctors, and patient care assistants were recruited.
- Checklist:
The compliance rate was increased from 79% in simulation to 98% in onsite audit.
- Preparation:
Participants demonstrated 100% compliance with equipment preparation, utilization of oxygen cylinder; Compliance with medication preparation was increased from 70% in simulation to 100% in audits.
- Communication:
Participants maintained 100% compliance on effective communication with transport destinations; Compliance on clear role delegation was improved from 70% in simulation to 100% in onsite audits.
- Situational awareness:
Participants demonstrated 100% compliance on reviewing patients’ hemodynamic status and tackling existing alarms.
2.Review: Regular transportation audit
Doctors, nurses and patient care assistants were invited.
Participants maintained 100% compliance on equipment preparation, effective communication, and situational awareness. The compliance rate of the transportation checklist increased from 98% in the previous audit to 100% in the recent transport audit.
All participants agreed the training program fosters teamwork among different disciplines, enhances their situational awareness and confidence in emergency response. A regular simulation training will be carried out to promote intrahospital transportation safety.
