Introduction :
Nurses play a crucial role in patient care. Continuous professional development is essential to keep their knowledge up to date amid evolving healthcare demands nowadays. In a rehabilitation hospital, like TWEH, simulation training may be limited due to resource constraints. However, scenario-based training offers a practical method for preparing nurses to meet the modern healthcare challenges. This can enhance nurses' competencies in an imitative and controlled clinical environments, allow them to experience some critical and high-risk situations that they may not encounter. Therefore, Nursing Services Division, Department of Medicine and Rehabilitation and Quality and Safety Office have collaborated to provide Scenario-Based Workshops to our nurses.
Objectives :
-To develop local courseware on Scenario-Based Workshops to equip nurses with knowledge and skills in rehabilitation nursing
-To increase awareness to different situations impeding operations in rehabilitation hospital setting
-To understand the principle on making the situation judgement and manage immediate risk affecting operation in a team approach
Methodology :
Firstly, we identified common situations occurring in our rehabilitation hospital to serve as the foundation for our training scripts. Then, we created a controlled training environment using a manikin that responds to participants’ actions. Trainers could instantly adjust patients’ parameters, such as vital signs and ECG patterns, based on reactions and scenario developments. Participants acted as doctors, PCAs, and patients in the scenarios, enhancing their understanding of others' actions and patients' feelings during critical situations. This approach enhanced self-reflection, teamwork, and improved patient-centric services. At the end of each scenario, a debriefing session allowed participants to reflect on their actions and learn how to handle similar situations more effectively in real life. The Crew Resource Management (CRM) concept was incorporated to improve communication, critical thinking and decision-making. Additionally, senior nurses from the clinical side joined as trainers to share their practical tips for various situations. Through simulating real-life clinical scenarios, participants engaged in experiential learning.
Result & Outcome :
From 2023 to 2024, eight scenario-based workshops were conducted, focusing on common high-risk scenarios such as managing unstable hemodynamics, inter-hospital transfer of critically ill patients, and troubleshooting ventilator alarms. A total of 79 nurses participated in the training. Evaluation results indicated overall satisfaction score was 5.47/6 and participants reported the content is practical for use in their workplace (score 5.51/6). Scenario-based training workshops organized locally effectively meets the training needs of frontline nurses. It increases awareness of patient deterioration and provides opportunities to manage challenging situations in a controlled environment before encountering them in real patient care. In the future, the assistance from clinical experts, such as, nurse consultants, would be sought to ensure our training scenarios remain up-to-date and relevant to evolving clinical situations.
