Objectives :
To understand the demography of our patients in GDH, FYK and MMRC. How a patient / carer would handle when an issue or concern arises with medications? Who to consult when they have a medication query? To explore the patient’s understanding of HA Go. Provide prospective pharmacy student the understanding of the role of pharmacist.
Methodology :
Engaged 5 Student health ambassadors and 1 mentor from Medical Social Service (MSS) to assist in the project. A survey with 11 questions was designed. Pharmacist liaise with Ward Manager and Volunteer coordinator to create a daily discharge list. The coordinator then refer discharge cases of GDH, wards 1-4 on FYKH, GW and NSW to our student ambassadors. The survey results are then analyze and review for area of improvement by pharmacy department If required, technical questions are to refer back to Pharmacist. Refer questions to pharmacist 5 student health ambassadors and 1 mentor of medical social worker as coordinator in the project The survey results are then analyze and review for area of improvements
Result & Outcome :
Survey period 25th Jul to 9th Aug 2024. To survey discharge patients from FYK and MMRC wards. The return rate of surveys. The understanding of use of HA Go. Improvement plans are formulated if any based on outcome.
