Introduction :
KWC Community Health Care (CHC) was established in 2022 to address today’s healthcare challenges. The success of the pilot service model led to the expansion of several KWC MSC pilot programs in 2022-23, including Breast cancer, Head and neck cancer survivorship, Medication Counselling Services, Adult Chronic Heart Failure, and Obesity and Weight Management. An advanced MSC service model was developed in 2024 to foster sustainable collaborations with different hospital teams.
Objectives :
1. Establish a cross-team co-location office for enhanced communication and efficiency.

2. Optimize hospital space and resources.

3. Cultivate a collaborative culture between KWC CHC and other clinical teams.
Methodology :
KWC GenesisHub in 7/F, Blk. G of Princess Margaret Hospital has renovated and opened in May 2024. Our teams aimed to help people staying health in the community and the Hub was designed for enhancing the communication and efficiency which divided into 4 zonings



Synergy Hub, Wellness Hub, Resources Hub & Knowledge Hub

In synergy hub, it co-located 7 KWC teams, CHC, MSC, Patient resources center (PRC), Community Nursing Service Liaison, Palliative care consultation, Psychosocial support service for cancer patient and Public-Private Partnership Office which promote teams’ interaction. The teams are keep posted by the regular cross teams meeting.

In the other hubs, community resources were in placed which allow to partition for different empowerment programs. With the PRC team on site, patients, carers could be navigated to community easily. Over 98% users found the GenesisHub helpful and would recommend to others.



Facilitation strategies

In addition to physical co-location, KWC CHC has launched quarterly webinars and tours in the MedicoSocialtrio Program 2024, with over a 98% satisfaction rate, to promote a collaborative culture. This model has been well-received and has led to more pilot programs, including Partnership with the Hong Kong Medical Association, chronic pain, Cardiac Rehabilitation, Inflammatory bowel disease, Palliative Day Care Service, Chinese - Western Medicine Pilot on Diabetes, CAPD Care in Residential Care Homes with positive feedback.

Conclusion and Wayforward

The advanced service model has proven effective and sustainable in meeting the growing demand for MSC. The way forward involves extending collaborations to other clinical teams within KWC.
