Neonatal Resuscitation Training in A&E Department to Increase Staff Confidence in Neonatal Stabilization and Resuscitation

Introduction :
Neonatal resuscitation is infrequent but high-stakes event for A&E department doctors and nurses, which required specific skills and preparation when delivery occurs before or arrived in the A&E department. Staff training is essential to provides the knowledge and skill to excel in their roles and effectively to our practice. However, 67.6% of staff included A&E doctors and nurses in Kwong Wah Hospital never attended neonatal resuscitation program provider course (NRP), 24.3% of staff attend NRP over 2 years ago. Only 8.1% of staff attend within 2 years.
Objectives :
To design neonatal resuscitation simulation workshop in the A&E department to enhance their staff knowledge and skill, also increase their confidence in neonatal stabilization and resuscitation.
Methodology :
21 identical simulation workshops have been completed from 9-9-2024 to 17-12-2024. Pre-reading material was sent to all A&E doctors and nurses before the workshop. Each workshop was 1.5 hrs. and had 3-4 participants, total trained 81 staff which included WM: 3, ANC: 1, RN: 44, APN: 21 and doctor: 12. Inclusion criteria: all A&E residents and nurses. Exclusion criteria: First year graduate nurses, they will attend another 4 hrs. neonatal resuscitation training workshop. This workshop was held in A&E resuscitation room by NRP instructor. Workshop content included equipment familiarization of neonatal e-trolley, resuscitation flow chart, initial step and patient temperature control, positive pressure ventilation (PPV) technique, mask selection, MRSOPA corrective step, chest compression, Video intubation, assist intubation and fixation, UVC insertion and fixation, blood transfusion and medication. It also included 2 scenarios: Born Before Arrival (BBA) and Born on Arrival (BOA). After the workshop, staff need to complete evaluation form and rating their confidence of neonatal resuscitation. Use Paired t-test (two-tail) to compare the result. A&E Neonatal resuscitation drill has been held in Dec. for assessment by 2 NRP instructors including neonatologist and neonatal care nurse consultant.
Result & Outcome :
A total of 74 evaluations have been received. Confidence to perform neonatal stabilization and resuscitation before and after workshop mean was 2.135 and 4.838 respectively (P < 0.001, paired two-tail t-test). 93.2% of staff recommend holding this workshop yearly to review NRP knowledge and hand on resuscitation skills.
Continuous improvement and innovation in training programs will maintain standards in neonatal resuscitation and ensure staff are prepared to provide best care possible.
