Introduction :
The unfamiliar and uncomfortable environment with higher use of invasive catheters in PICU can be a triggering factor of paediatric delirium. Early detection of delirium is crucial, as it associated with prolonged hospitalization, increased morbidity and long-term cognitive deficits if not addressed. However, there is no paediatric delirium screening implemented in QMH PICU setting.
The CAPD scale is a validated screening tool for children 0-21 years of age. Through observational assessment and standardized rating scale, CAPD promote early detection of patients at risk of delirium. Therefore, an educational programme of paediatric delirium and CAPD scale was started in December 2023.
Objectives :
(1) To achieve a 100% participation rate in the educational sessions.
(2) To achieve at least 30% increase in the mean score of post-test compared to pre-test.
(3) All PICU nurses will rate confident/very confident to use CAPD scale
(4) Implementation of CAPD scale in PICU by August 2024 after the completion of educational program
Methodology :
Educational programme covered: (1) Basic knowledge of paediatric delirium (2) CAPD scale application (3) Nursing management on preventing the worsening of delirium symptoms.
We adopted pretest posttest design to evaluate the improvement of staff knowledge after the educational programme. Also, staff were required to rate their level of confidence in using CAPD on 5-point Likert scale which measured the self-efficacy of nurses in identifying delirium patients with the use of CAPD scale.
Result & Outcome :
All clinical nursing staffs enrolled in the educational programme and the average post-test score was 40% higher than the average pre-test score. After the educational program, 100% of the staffs scored confident or very confident to use CAPD scale. Routine delirium screening using the CAPD scale was initiated in the PICU in August 2024.
The educational programme enhanced nurses’ knowledge on paediatric delirium and their confidence level in using CAPD scale. It is believed that the programme could help the staff to early detect patients at-risk of delirium in PICU setting by using CAPD scale, in order to facilitate timely management and eventually improve the patient’s outcome.
