Introduction :
In recent months, we have observed an increasing trend in incidents related to T&S (Type and Screen) specimen handling by interns. Those Incidents mainly focus on incomplete specimen documentation e.g., forgotten signatures, missing request forms, incorrect specimen forms, and sending to wrong laboratories etc. Despite unnecessary repeated collections, this human error also posing risk on cases requiring urgent blood transfusion. Timely improvement is essential in securing patient safety and quality care delivery.
Objectives :
The primary objective is to enhance the safety and efficiency of the T&S sample handling and sending process by interns, with standardized checklist and verification system to ensure the correctness of specimen handling. We aim to reduce the incidence of non-compliance and ensure timely patient care.
Methodology :
1. Full analysis of T&S related incidents in 2024 and develop a checklist. This checklist was created for interns’ final check before sending out the specimen. Items include: Correct specimen labelling? Label with signature? Transfusion request form attached? Nurse’s countercheck? 2. Nursing Staff Involvement - To enhance accountability, nursing staff are required to check and countersign the checklist before the specimen is dispatched. This collaborative approach fosters teamwork and shared responsibility 3. Visual Reminders - A plastic cover with an eye-catching logo was designed and put on top of specimen collection box as a final reminder. This serves as a visual cue to ensure all checklist items have been addressed.
Result & Outcome :
Upon the implementation of this new strategies, we have achieved a full compliance on safe and accurate specimen sending. Both interns and nurses showed an increased in awareness of accurate specimen collection and sending. This CQI initiative has successfully built a safe T&S concept among the team. By implementing the checklist, visual reminders and nurses countersign, full compliance on T&S specimen handling is assured. This is indeed one step forward to promote patient safety. Future plan include further expansion to its use to all O&G wards to maintain high standard of patient care
