Introduction :
Continuous Ambulatory Peritoneal Dialysis (CAPD) is the most common treatment modality for patients with end stage renal failure in Hong Kong. When a patient is incompetent in CAPD care and lack of a helper at home, he/she might move to a RCHE for continuity of care. RCHE carers play a vital role to follow CAPD procedures strictly and avoid complications. Recently, the number of RCHEs in Kwai Tsing district has increased by 30%. In view of the unavailability of updated RCHE information with CAPD care for discharge planning and suboptimal CAPD care due to unknown standard in RCHEs, KWC Community Health Care team collaborated with PMH Renal team and Medical Social Services Unit to enhance CAPD care in RCHEs and reduce hospital utilization.
Objectives :
(1) To enhance RCHE carers’ knowledge and skills in CAPD care (2) To assess and identify RCHEs with up-to-standard CAPD care
Methodology :
A workgroup was formulated with all parties in April 2024 to work out the enhancement plan. This pilot model promotes patient-centered care and engages all stakeholders from different sectors, including Licensing & Regulation Branch of Social Welfare Department, Patient Group, Renal Mutual Help Association and RCHE superintendents. Educational talk was conducted via webinar in September 2024. All RCHEs in Kwai Tsing and Tsuen Wan districts were invited. CAPD patients under PMH Renal would be visited with all involved sectors. Onsite assessment of RCHE environment and CAPD procedure would be assessed with checklist. For those RCHEs not complying to standard, carers would have to attend the refresher training on CAPD care.
Result & Outcome :
105 RCHEs (99%) with 262 staff attended the educational talk. Knowledge of RCHE carers was improved with mean score of knowledge test improved by 19% after the talk. CAPD Patient and RCHE Visit was conducted in 12 RCHEs from October 2024 to March 2025. Refresher training was organized for 1 RCHE to enhance their skills in CAPD care. Assessment results of visit, participants’ satisfaction level and CAPD-related hospital utilization would be analyzed for future implementation improvement. This innovative model of MSC on CAPD care is expected to be rolled out to other hospitals which would be beneficial to more CAPD patients in Hong Kong.
