Introduction :
With the concept of digital health, a self-help digital logbook had been established by multi-disciplinary team including psychiatrist, psychiatric nurse, occupational therapist, social worker and clinical psychologist. By utilization of HA drive, digital contents of the individualized logbook by multidisciplinary staff can be retrieved for interview sessions to engage patients in their detoxification journey among hospitalized and community services. Digital logbook had started to implement since 9/2024 in TPH psychiatric wards, which was a first-ever mode of utilization across hospitals.
Objectives :
1. Enhance patient’s self-efficacy in detoxification journey 2. Boost up patient’s motivation to achieve positive changes 3. Facilitate effective communication and case discussion among multidisciplinary team
Methodology :
9 sets of mobile tablets were used for operating the digital logbook with node-link mapping drug cycle, self-written worksheets with psychoeducational information, self-rating tools and diary. Multidisciplinary team contributed different structural inputs with patients interactively. Each patient was assigned with a QR code of own’s digital logbook for continuation of self-help mode as well as enhancing strength on personal skills, throughout their detoxification journey. Digital logbook supported the clinical work of multidisciplinary team, like facilitating case discussion in weekly meeting, supportive monitoring by community partners including community psychiatric services and substance abuse clinic. Data from assessment tools of SCORATES, Drug Avoidance Self-Efficacy Scale (DASES), and satisfaction survey of patients and staff were evaluated after 4 months’ period.
Result & Outcome :
From 1/9/24 to 31/12/24, 28 patients were recruited for the use of digital logbook and valid for further evaluation on assessment and satisfaction survey. In SOCRATES, significant effects in enhancing readiness to change in substance were resulted (p<0.001, pair-test). Strongest effect of the intervention was observed in the “Recognition” scale (d=0.75), while “Ambivalence” scale (d= 0.53) and “taking Step” scale (d=0.47) also showed a significant effect. In DASES, 100% patients (n=28, N=28) rated their level of confidence to resist drug use showed positive change, with self-efficiency from low level to moderate or even high level. In satisfaction survey, 100% patients (n=28, N=28) agreed digital logbook could enhance their self-motivation and self-awareness. 100% multidisciplinary staff (n=45, N= 45) showed satisfaction to the digital health approach for communication and case discussion during patient journey. Synergy can be created.
